Variant summary: TMEM67 c.1538A>G (p.Tyr513Cys) results in a non-conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 8e-06 in 250304 control chromosomes. c.1538A>G has been reported in the literature in multiple individuals affected with Joubert Syndrome And Related Disorders (e.g. Baala_2007, Doherty_2010, Halbritter_2013, Summers_2016, Tallila_2009). These data indicate that the variant is very likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 17160906, 19466712, 23559409, 19574260, 28497568). ClinVar contains an entry for this variant (Variation ID: 1371). Based on the evidence outlined above, the variant was classified as pathogenic. -

This sequence change replaces tyrosine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 513 of the TMEM67 protein (p.Tyr513Cys). This variant is present in population databases (rs137853107, gnomAD 0.002%). This missense change has been observed in individual(s) with Joubert syndrome (PMID: 17160906, 28497568). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 1371). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt TMEM67 protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic. -
