Variant summary
Our verdict is Uncertain significance. The variant received 5 ACMG points: 5P and 0B. PM2PP3_ModeratePP5
The NM_017890.5(VPS13B):c.8318C>T(p.Ser2773Leu) variant causes a missense change. The variant allele was found at a frequency of 0.0000103 in 1,461,814 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Synonymous variant affecting the same amino acid position (i.e. S2773S) has been classified as Likely benign.

Cohen syndrome Pathogenic:1Uncertain:1
This sequence change replaces serine, which is neutral and polar, with leucine, which is neutral and non-polar, at codon 2773 of the VPS13B protein (p.Ser2773Leu). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with Cohen syndrome (PMID: 16648375). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 68093). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt VPS13B protein function with a positive predictive value of 80%. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -
