The G544V missense variant in the STXBP1 gene has been reported previously as a de novo pathogenic variant in an individual with neonatal epileptic encephalopathy with evolution to West syndrome (Weckhuysen et al., 2013). Missense variants in at the same and nearby residues (G544C, G544D, C552R) have been reported in the Human Gene Mutation Database in association with STXBP1-related disorders (Stenson et al., 2014), supporting the functional importance of this region of the protein. The G544V variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. This conservative amino acid substitution occurs at a position that is conserved across species, and in silico analysis predicts this variant is probably damaging to the protein structure/function. Therefore, G544V is interpreted as a pathogenic variant.

This sequence change replaces glycine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 544 of the STXBP1 protein (p.Gly544Val). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with epileptic encephalopathy (PMID: 23409955, 28628100, 29314583). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 429757). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt STXBP1 protein function. This variant disrupts the p.Gly544 amino acid residue in STXBP1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 18469812, 23934111, 26514728, 29186148). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
