Variant summary
Our verdict is Uncertain significance. Variant got 4 ACMG points: 4P and 0B. PM2PP3_Moderate
The NM_017617.5(NOTCH1):c.3974C>A(p.Ala1325Asp) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Synonymous variant affecting the same amino acid position (i.e. A1325A) has been classified as Likely benign.

In summary, this variant is a novel missense change with uncertain impact on protein function. It has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies. This variant is not present in population databases (ExAC no frequency) and has not been reported in the literature in individuals with a NOTCH1-related disease. This sequence change replaces alanine with aspartic acid at codon 1325 of the NOTCH1 protein (p.Ala1325Asp). The alanine residue is highly conserved and there is a moderate physicochemical difference between alanine and aspartic acid. -
Familial thoracic aortic aneurysm and aortic dissection Uncertain:1
The c.3974C>A (p.A1325D) alteration is located in exon 24 (coding exon 24) of the NOTCH1 gene. This alteration results from a C to A substitution at nucleotide position 3974, causing the alanine (A) at amino acid position 1325 to be replaced by an aspartic acid (D). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
not provided Uncertain:1
Has not been previously published as pathogenic or benign to our knowledge; Not observed in large population cohorts (Lek et al., 2016); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Reported in ClinVar as a variant of uncertain significance (ClinVar Variant ID# 409072; Landrum et al., 2016) -
Aortic valve disease 1 Uncertain:1
