Variant summary: CDKN2A c.400G>C (p.Ala134Pro) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 4.1e-06 in 245336 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.400G>C has been reported in the literature as a VUS in settings of multigene cancer panel testing in at-least one individual with endometrial cancer (Ring_2016) and as a germline rare coding variant (RCV) in a cohort of controls (1/900, African American/African) but not in cases with pancreatic cancer (0/220) (McWilliams_2018). These report(s) do not provide unequivocal conclusions about association of the variant with Cutaneous Malignant Melanoma. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Two clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -
Hereditary cancer-predisposing syndrome Uncertain:1
The p.A134P variant (also known as c.400G>C), located in coding exon 2 of the CDKN2A gene, results from a G to C substitution at nucleotide position 400. The alanine at codon 134 is replaced by proline, an amino acid with highly similar properties. In a study examining the role of CDKN2A in pancreatic cancer, this variant was seen in 1/900 African American controls but not in any pancreatic cancer cases (McWilliams RR et al. Cancer Epidemiol. Biomarkers Prev., 2018 11;27:1364-1370). This alteration has also been detected in a cohort of 381 unselected endometrial cancer patients who underwent multi-gene panel testing and classified as a variant of uncertain significance by the authors (Ring KL et al. Mod Pathol, 2016 11;29:1381-1389). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

This sequence change replaces alanine, which is neutral and non-polar, with proline, which is neutral and non-polar, at codon 134 of the CDKN2A (p16INK4a) protein (p.Ala134Pro). This variant is present in population databases (rs372599739, gnomAD 0.007%). This missense change has been observed in individual(s) with endometrial caner (PMID: 27443514). ClinVar contains an entry for this variant (Variation ID: 495536). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
