The p.T79S variant (also known as c.236C>G), located in coding exon 2 of the CDKN2A gene, results from a C to G substitution at nucleotide position 236. The threonine at codon 79 is replaced by serine, an amino acid with similar properties. This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.

In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site, but this prediction has not been confirmed by published transcriptional studies. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of p.His93Gln in p14ARF (SIFT: "Tolerated"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). These same algorithms all suggest that p.Thr79Ser in p16INK4a is likely to be tolerated. The serine amino acid residue at this position in p16INK4a is found in multiple mammalian species, suggesting that this missense change does not adversely affect protein function. These predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with CDKN2A-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change replaces threonine with serine at codon 79 of the CDKN2A (p16INK4a) protein (p.Thr79Ser). The threonine residue is highly conserved and there is a small physicochemical difference between threonine and serine. Alternatively, this sequence change replaces histidine with glutamine at codon 93 of the CDKN2A (p14ARF) protein (p.His93Gln). The histidine residue is moderately conserved and there is a small physicochemical difference between histidine and glutamine. The CDKN2A gene encodes two different proteins, p16INK4a and p14ARF, which are translated from alternative transcripts that have different open reading frames.
