Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt FANCC protein function with a negative predictive value of 80%. It is also reported in unaffected individuals (PMID: 33471991 (2021), see also LOVD (https://databases.lovd.nl/shared/variants/FANCC)). The frequency of this variant in the general population, 0.000008 (2/251148 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is benign or damaging. The FANCC c.472G>C variant is predicted to result in the amino acid substitution p.Ala158Pro. This variant was identified in two patients undergoing testing for hereditary breast and ovarian cancer (Lerner-Ellis et al. 2020. PubMed ID: 32885271), as well as in five individuals with breast cancer and two healthy control individuals (Breast Cancer Association Consortium et al. 2021. PubMed ID: 33471991). This variant is reported in 0.0054% of alleles in individuals of East Asian descent in gnomAD and has been interpreted as either likely benign or uncertain in ClinVar (https://www.ncbi.nlm.nih.gov/clinvar/variation/234311). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -

The variant was identified in dbSNP (ID: rs372338418) as "With Uncertain significance allele" and ClinVar (classified as uncertain significance by GeneDx, Invitae, and Ambry Genetics). The variant was identified in control databases in 2 of 245948 chromosomes at a frequency of 0.000008 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the following populations: Latino in 1 of 33540 chromosomes (freq: 0.00003) and East Asian in 1 of 17212 chromosomes (freq: 0.00006), while the variant was not observed in the African, Other, European, Ashkenazi Jewish, Finnish, or South Asian populations. The p.Ala158 residue is not conserved in mammals and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) provide inconsistent predictions regarding the impact to the protein; this information is not very predictive of pathogenicity. The variant occurs outside of the splicing consensus sequence and 2 of 4 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) predict a greater than 10% difference in splicing; this is not very predictive of pathogenicity. In summary, based on the above information, the clinical significance of this variant cannot be determined with certainty at this time.
