Variant summary
Our verdict is Likely benign. The variant received -4 ACMG points: 1P and 5B. PP3BP6BS2
The NM_000264.5(PTCH1):c.3436G>A(p.Asp1146Asn) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000991 in 1,613,962 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Synonymous variant affecting the same amino acid position (i.e. D1146D) has been classified as Likely benign.

Hereditary cancer-predisposing syndrome Uncertain:1
The p.D1146N variant (also known as c.3436G>A), located in coding exon 20 of the PTCH1 gene, results from a G to A substitution at nucleotide position 3436. The aspartic acid at codon 1146 is replaced by asparagine, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -
