The NM_000249.4(MLH1):c.793C>G (p.Arg265Gly) variant causes a missense, splice region change involving the alteration of a conserved nucleotide. The variant is absent from the gnomAD population database at sites with sufficient sequencing coverage. In-silico predictor (REVEL) classifies this variant as likely damaging/oncogenic. Splicing prediction tools (SpliceAI) predict no significant impact on normal splicing. The affected nucleotide is highly conserved across species (PhyloP 100-way vertebrate score: 7.28). Variant has been reported in ClinVar as Pathogenic/Likely Pathogenic (★★). ClinVar reports functional evidence for this variant: "SCV002676419: Wanat JJ et al. Hum. Mol. Genet. 2007 Feb;16:445-52" and additional evidence is available in ClinVar. A different missense at the same amino acid position has been reported as Pathogenic/Likely Pathogenic in ClinVar: p.R265V: Pathogenic (ClinVar VariationId 619508, 1 star) Other variants at the same amino acid position have been reported in ClinVar (not pathogenic): p.R265H: Conflicting_classifications_of_pathogenicity (ClinVar VariationId 90381); p.R265L: Uncertain_significance (ClinVar VariationId 1928288, 2 stars); p.R265= (synonymous): Likely_benign (ClinVar VariationId 491730, 2 stars)

Genes affected
MLH1 (HGNC:7127): (mutL homolog 1) The protein encoded by this gene can heterodimerize with mismatch repair endonuclease PMS2 to form MutL alpha, part of the DNA mismatch repair system. When MutL alpha is bound by MutS beta and some accessory proteins, the PMS2 subunit of MutL alpha introduces a single-strand break near DNA mismatches, providing an entry point for exonuclease degradation. The encoded protein is also involved in DNA damage signaling and can heterodimerize with DNA mismatch repair protein MLH3 to form MutL gamma, which is involved in meiosis. This gene was identified as a locus frequently mutated in hereditary nonpolyposis colon cancer (HNPCC). [provided by RefSeq, Aug 2017]

PS3Functional studies show damaging effect
Well-established functional study supports damaging effect (PS3); SCV002676419: Wanat JJ et al. Hum. Mol. Genet. 2007 Feb;16:445-52; Drost M et al. Hum. Mutat. 2010 Mar;31:247-53; van der Klift HM et al. Mol Genet Genomic Med. 2015 Jul;3:327-45; Soukarieh O et al. PLoS Genet. 2016 Jan;12:e1005756.; SCV003786423: Studies have shown that this missense change results in skipping of exon 10, and produces a non-functional protein and/or introduces a premature termination codon (internal data).; SCV004185539: Functional studies indicate this variant impacts protein function [Myriad internal data].; SCV001363159: Internal RNA analysis reports experimental evidence that this variant affects mRNA splicing resulting in skipping of exon 10 and exon 10-11 in a smaller proportion of transcripts, each of which would result in an out-of frame outcome (internal data).
