The NM_000264.5(PTCH1):c.884C>A (p.Pro295His) variant causes a missense change involving the alteration of a conserved nucleotide. The gene PTCH1 is a tumor suppressor gene (CancerMine: 35 TSG, 20 oncogene, 13 driver citations). The gene PTCH1 is a known oncogene (CancerMine: 35 TSG, 20 oncogene, 13 driver citations). The gene PTCH1 is a cancer driver gene (CancerMine: 35 TSG, 20 oncogene, 13 driver citations). The variant is absent from the gnomAD population database at sites with sufficient sequencing coverage. In-silico predictor (REVEL) classifies this variant as likely damaging/oncogenic. Splicing prediction tools (SpliceAI) predict no significant impact on normal splicing. The affected nucleotide is highly conserved across species (PhyloP 100-way vertebrate score: 9.60). No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar. Other variants at the same amino acid position have been reported in ClinVar (not pathogenic): p.P295L: Conflicting_classifications_of_pathogenicity (ClinVar VariationId 135908); p.P295= (synonymous): Likely_benign (ClinVar VariationId 1764907, 1 star); p.P295= (synonymous): Likely_benign (ClinVar VariationId 2916215, 2 stars); p.P295S: Uncertain_significance (ClinVar VariationId 2633768, 2 stars); p.P295T: Uncertain_significance (ClinVar VariationId 4862716, 1 star) The variant has been observed in cBioPortal in 1 sample across 1 study and 1 cancer type; somatic enrichment level: NONE (Observed in at most one deduplicated cBioPortal case.).

Genes affected
PTCH1 (HGNC:9585): (patched 1) This gene encodes a member of the patched family of proteins and a component of the hedgehog signaling pathway. Hedgehog signaling is important in embryonic development and tumorigenesis. The encoded protein is the receptor for the secreted hedgehog ligands, which include sonic hedgehog, indian hedgehog and desert hedgehog. Following binding by one of the hedgehog ligands, the encoded protein is trafficked away from the primary cilium, relieving inhibition of the G-protein-coupled receptor smoothened, which results in activation of downstream signaling. Mutations of this gene have been associated with basal cell nevus syndrome and holoprosencephaly. [provided by RefSeq, Aug 2017]

Classification according to ACGS-UK Somatic Oncogenicity v2025

Classification was made for transcript:

Our verdict: Uncertain_significance. The variant received 3 points.

O3Absent or very rare in population
Absent or extremely rare in gnomAD (AC ≤ 2) — O3 moderate; GnomAD: AF = absent, AC = 0 — absent/extremely rare (O3 moderate [+2])
O6Computational evidence supports deleterious effect
Computational evidence supports a deleterious/oncogenic effect; Missense variant — primary computational scorer supports an oncogenic/deleterious effect
