Mutations in this gene are associated with X-linked Alport syndrome, also known as hereditary nephritis. Like the other members of the type IV collagen gene family, this gene is organized in a head-to-head conformation with another type IV collagen gene so that each gene pair shares a common promoter. Affects a glycine residue in a Gly-X-Y motif in the triple helical region of the COL4A5 gene, where the majority of pathogenic missense variants occur; this glycine flanks an interruption within the triple helical region. Missense substitution of a canonical Glycine residue in a Gly-X-Y motif in the triple helical region often disrupts normal protein folding and function; however this particular collagen gene is known to have several naturally occurring interruptions of the Gly-X-Y repeats and we cannot precisely predict the consequences of this change; In silico analysis, which includes protein predictors and evolutionary conservation, supports a deleterious effect; Not observed in large population cohorts (Lek et al., 2016); Has not been previously published as pathogenic or benign to our knowledge -
