This missense change has been observed in individuals with clinical features of Alport syndrome (PMID: 8940267, 22921432; Invitae). This sequence change replaces serine with asparagine at codon 1659 of the COL4A5 protein (p.Ser1659Asn). The serine residue is highly conserved and there is a small physicochemical difference between serine and asparagine. RNA analysis indicates that this missense change induces altered splicing and likely disrupts the C-terminus of the protein. This variant is not present in population databases (ExAC no frequency). This variant is also known as 5178G>A. ClinVar contains an entry for this variant (Variation ID: 24782). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Studies have shown that this missense change results in skipping of exon 50 and introduces a new termination codon (PMID: 8940267). However the mRNA is not expected to undergo nonsense-mediated decay. For these reasons, this variant has been classified as Pathogenic.
