NM_000132.3(F8):c.43C>T(R15*) is a nonsense variant classified as pathogenic in the context of hemophilia A. R15* has been observed in cases with relevant disease (PMID: 39260745, 32026663, 20533009). Relevant functional assessments of this variant are not available in the literature. R15* has not been observed in referenced population frequency databases. In summary, NM_000132.3(F8):c.43C>T(R15*) is nonsense variant in a gene where loss of function is a known mechanism of disease, is predicted to disrupt protein function, and has been observed more frequently in cases with the relevant disease than in healthy populations. Please note: this variant was assessed in the context of healthy population screening.
The variant is not observed in the gnomAD v2.1.1 dataset. This variant was predicted to result in a loss or disruption of normal protein function through nonsense-mediated decay (NMD) or protein truncation. Multiple pathogenic variants are reported downstream of the variant. The variant has been reported at least twice as pathogenic without evidence for the classification (ClinVar ID: VCV000010152 / PMID: 1979502). Therefore, this variant is classified as Pathogenic according to the recommendation of ACMG/AMP guideline.

The F8 c.43C>T; p.Arg15Ter variant (rs387906432), also known as -5 in legacy nomenclature, is reported in the literature in individuals with severe hemophilia A (Bogdanova 2005, Feng 2021, Pattinson 1990). This variant is also reported in ClinVar (Variation ID: 10152). It is absent from the Genome Aggregation Database, indicating it is not a common polymorphism. This variant induces an early termination codon and is predicted to result in a truncated protein or mRNA subject to nonsense-mediated decay. Based on available information, this variant is considered to be pathogenic. References: Bogdanova N et al.
