Genes affected
PHEX (HGNC:8918): (phosphate regulating endopeptidase X-linked) The protein encoded by this gene is a transmembrane endopeptidase that belongs to the type II integral membrane zinc-dependent endopeptidase family. The protein is thought to be involved in bone and dentin mineralization and renal phosphate reabsorption. Mutations in this gene cause X-linked hypophosphatemic rickets. Alternative splicing results in multiple transcript variants. [provided by RefSeq, Sep 2013]
PTCHD1-AS (HGNC:37703): (PTCHD1 antisense RNA (head to head))

Our verdict: Pathogenic. The variant received 17 ACMG points.

PM2
Very rare variant in population databases, with high coverage;
PM5
Other missense variant is known to change same aminoacid residue: Variant chrX-22247901-G-C is described in ClinVar as Pathogenic. ClinVar VariationId is 438487.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.
PP2
Missense variant in the gene, where a lot of missense mutations are associated with disease in ClinVar. The gene has 135 curated pathogenic missense variants (we use a threshold of 10). The gene has 25 curated benign missense variants. Gene score misZ: 1.7091 (below the threshold of 3.09). Trascript score misZ: NaN (below the threshold of 3.09). GenCC associations: The gene is linked to X-linked dominant hypophosphatemic rickets.

This variant is predicted to substitute a cysteine residue by an phenylalanine residue. This variant is not found in Genome Aggregation Database (v2.1.1). This variant has been reported in the literature (PMID 33639975) in individuals with a diagnosis of X-linked hypophosphatemic rickets. Computational tools indicate this change is damaging. Based on the ACMG variant interpretation guidelines, the available evidence supports classification of this variant as pathogenic.

ClinVar contains an entry for this variant (Variation ID: 803777). For these reasons, this variant has been classified as Pathogenic. This variant disrupts the p.Cys733 amino acid residue in PHEX. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 2589938, 22527485). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt PHEX protein function. This missense change has been observed in individual(s) with clinical features of hypophosphatemic rickets (PMID: 33639975). This variant is not present in population databases (gnomAD no frequency). This sequence change replaces cysteine, which is neutral and slightly polar, with phenylalanine, which is neutral and non-polar, at codon 733 of the PHEX protein (p.Cys733Phe).
