Variant summary
Our verdict is Likely benign. The variant received -4 ACMG points: 0P and 4B. BP4_ModerateBP6BS2_Supporting
The NM_004006.3(DMD):c.6452G>A(p.Gly2151Asp) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.000199 in 1,208,547 control chromosomes in the GnomAD database, with no homozygous occurrence. There are 79 hemizygotes in GnomAD. In-silico tool predicts a benign outcome for this variant. 11/19 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Synonymous variant affecting the same amino acid position (i.e. G2151G) has been classified as Likely benign.

The p.G2151D variant (also known as c.6452G>A), located in coding exon 45 of the DMD gene, results from a G to A substitution at nucleotide position 6452. The glycine at codon 2151 is replaced by aspartic acid, an amino acid with similar properties. This alteration has been reported in a dilated cardiomyopathy (DCM) cohort (Mazzarotto F et al. Circulation, 2020 Feb;141:387-398). Based on data from gnomAD, the A allele has an overall frequency of 0.004% (8/182145) total alleles studied, with 3 hemizygotes observed. The highest observed frequency was 0.02% (3/13116) of African alleles. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
