Genes affected
ERLIN2 (HGNC:1356): (ER lipid raft associated 2) This gene encodes a member of the SPFH domain-containing family of lipid raft-associated proteins. The encoded protein is localized to lipid rafts of the endoplasmic reticulum and plays a critical role in inositol 1,4,5-trisphosphate (IP3) signaling by mediating ER-associated degradation of activated IP3 receptors. Mutations in this gene are a cause of spastic paraplegia-18 (SPG18). Alternatively spliced transcript variants encoding multiple isoforms have been observed for this gene. ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 12 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant located near the start codon (<100nt), not predicted to undergo nonsense mediated mRNA decay. There are 22 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 8-37737964-CT-C is Pathogenic according to our data. Variant chr8-37737964-CT-C is described in ClinVar as [Pathogenic]. Clinvar id is 639125.Status of the report is criteria_provided_single_submitter, 1 stars.

Spastic paraplegia Pathogenic:1
Pathogenic, criteria provided, single submitterclinical testingInvitaeDec 28, 2018This sequence change creates a premature translational stop signal (p.Cys16Valfs*11) in the ERLIN2 gene. It is expected to result in an absent or disrupted protein product. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with ERLIN2-related conditions. Loss-of-function variants in ERLIN2 are known to be pathogenic (PMID: 21330303, 23109145, 24482476, 27824013). For these reasons, this variant has been classified as Pathogenic. -
