Genes affected
PAFAH1B1 (HGNC:8574): (platelet activating factor acetylhydrolase 1b regulatory subunit 1) This locus was identified as encoding a gene that when mutated or lost caused the lissencephaly associated with Miller-Dieker lissencephaly syndrome. This gene encodes the non-catalytic alpha subunit of the intracellular Ib isoform of platelet-activating factor acteylhydrolase, a heterotrimeric enzyme that specifically catalyzes the removal of the acetyl group at the SN-2 position of platelet-activating factor (identified as 1-O-alkyl-2-acetyl-sn-glyceryl-3-phosphorylcholine). Two other isoforms of intracellular platelet-activating factor acetylhydrolase exist: one composed of multiple subunits, the other, a single subunit. In addition, a single-subunit isoform of this enzyme is found in serum. [provided by RefSeq, Apr 2009]

ACMG classification

Classification made for transcript

Verdict is Likely_pathogenic. Variant got 9 ACMG points.

PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 17-2672645-T-C is Pathogenic according to our data. Variant chr17-2672645-T-C is described in ClinVar as [Likely_pathogenic]. Clinvar id is 21182.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.
BP4
Computational evidence support a benign effect (BayesDel_noAF=-0.82). . Strength limited to SUPPORTING due to the PP5.

Pathogenic, criteria provided, single submitterclinical testingRady Children's Institute for Genomic Medicine, Rady Children's Hospital San Diego-This variant has been previously reported as a de novo change in patients with lissencephaly (PMID: 11115846, 27891766). Functional studies in patient fibroblasts demonstrated that this variant leads to exon skipping (PMID: 27891766). It is absent from the gnomAD population database and thus is presumed to be rare. Analysis of the parental samples was negative for the variant, indicating the variant likely occurred as a de novo event. Based on the available evidence, the c.569-10T>C variant is classified as Pathogenic. -

Pathogenic, criteria provided, single submitterclinical testingInvitaeAug 21, 2022For these reasons, this variant has been classified as Pathogenic. Studies have shown that this variant results in skipping of exon 7 and introduces a premature termination codon (PMID: 27891766). The resulting mRNA is expected to undergo nonsense-mediated decay. ClinVar contains an entry for this variant (Variation ID: 21182). This variant has been observed in individual(s) with lissencephaly (PMID: 11115846, 17664403, 27891766). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (gnomAD no frequency). This sequence change falls in intron 6 of the PAFAH1B1 gene. It does not directly change the encoded amino acid sequence of the PAFAH1B1 protein. RNA analysis indicates that this variant induces altered splicing and may result in an absent or disrupted protein product. -
