Nemaline myopathy Pathogenic:1
The c.2106+3A>C variant in NEB has been reported in 2 affected individuals with nemaline myopathy (PMID: 21350120, 33333461), and has been identified in 0.0003% (4/1179222) of European (non-Finnish) chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP ID: rs1553589058). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. This variant has also been reported in ClinVar (Variation ID: 555744) and has been interpreted as a variant of uncertain significance by Counsyl. Of the 2 affected individuals, one was a compound heterozygote that carried a reported pathogenic/likely pathogenic variant in trans, which increases the likelihood that the c.2106+3A>C variant is pathogenic (Variation ID: 190457; PMID: 21350120). RNAseq analysis performed on affected tissue shows skipping of exon 22 (PMID: 21350120). This variant is located in the 3’ splice region. Computational tools do suggest an impact to splicing. However, this information is not predictive enough to determine/rule out pathogenicity. Loss of function of the NEB gene is an established disease mechanism in autosomal recessive nemaline myopathy. The phenotype of an individual heterozygous for this variant is highly specific for nemaline myopathy based on the presence of nemaline rods on a muscle biopsy consistent with disease (PMID: 21350120). In summary, although additional studies are required to fully establish its clinical significance, this variant is likely pathogenic for autosomal recessive nemaline myopathy.
