SCN11A (HGNC:10583): (sodium voltage-gated channel alpha subunit 11) Voltage-gated sodium channels are transmembrane glycoprotein complexes composed of a large alpha subunit with 24 transmembrane domains and one or more regulatory beta subunits. They are responsible for the generation and propagation of action potentials in neurons and muscle. This gene encodes one member of the sodium channel alpha subunit gene family, and is highly expressed in nociceptive neurons of dorsal root ganglia and trigeminal ganglia. It mediates brain-derived neurotrophic factor-evoked membrane depolarization and is a major effector of peripheral inflammatory pain hypersensitivity. Mutations in this gene have been associated with hereditary sensory and autonomic neuropathy type VII and familial episodic pain syndrome-3. The c.1927A>G (p.I643V) alteration is located in coding exon 13 of the SCN11A gene. This alteration results from a A to G substitution at nucleotide position 1927, causing the isoleucine (I) at amino acid position 643 to be replaced by a valine (V). Based on data from the Genome Aggregation Databas (gnomAD), the SCN11A c.1927A>G alteration was observed in 0.003% (9/282,688) of total alleles studied. The p.I643 amino acid is poorly conserved in available vertebrate species, and several species have valine as the reference amino acid. The p.I643V alteration is predicted to be benign by Polyphen and tolerated by SIFT in silico analyses. Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
