Variant summary
Our verdict is Uncertain significance. Variant got 5 ACMG points: 5P and 0B. PM1PM2PP3
The NM_001048174.2(MUTYH):c.713C>G(p.Ala238Gly) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000274 in 1,461,794 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. A238V) has been classified as Uncertain significance.

Hereditary cancer-predisposing syndrome Uncertain:4
This missense variant replaces alanine with glycine at codon 266 of the MUTYH protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in unknown phase with p.Glu466del in an individual affected with attenuated colorectal polyposis (https://databases.lovd.nl/shared/individuals/00203816) and in an individual undergoing evaluation for inherited cancer predisposition (PMID: 31159747). In an international breast cancer case-control meta-analysis, this variant was detected in 1/60466 cases and 1/53461 controls (PMID: 33471991). This variant has been identified in 2/249976 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.A266G variant (also known as c.797C>G), located in coding exon 10 of the MUTYH gene, results from a C to G substitution at nucleotide position 797. The alanine at codon 266 is replaced by glycine, an amino acid with similar properties. This alteration was reported as a variant of unknown signifcance in 1/1197 individuals from Greece, Romania, and Turkey undergoing evaluation for inherited cancer predisposition (Tsaousis GN et al. BMC Cancer, 2019 Jun;19:535). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Familial adenomatous polyposis 2 Uncertain:3
This sequence change replaces alanine, which is neutral and non-polar, with glycine, which is neutral and non-polar, at codon 266 of the MUTYH protein (p.Ala266Gly). This variant is present in population databases (rs750344996, gnomAD 0.002%). This missense change has been observed in individual(s) with clinical features of MUTYH-related conditions (PMID: 31159747; internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 186750). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

not provided Uncertain:1
Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Observed in individuals with a personal or family history of breast and/or ovarian cancer, as well as in unaffected control groups (Tsaousis et al., 2019; Dorling et al., 2021); This variant is associated with the following publications: (PMID: 23108399, 11801590, 11092888, 11160897, 33471991, 31159747) -
