Genes affected
SF3B4 (HGNC:10771): (splicing factor 3b subunit 4) This gene encodes one of four subunits of the splicing factor 3B. The protein encoded by this gene cross-links to a region in the pre-mRNA immediately upstream of the branchpoint sequence in pre-mRNA in the prespliceosomal complex A. It also may be involved in the assembly of the B, C and E spliceosomal complexes. In addition to RNA-binding activity, this protein interacts directly and highly specifically with subunit 2 of the splicing factor 3B. This protein contains two N-terminal RNA-recognition motifs (RRMs), consistent with the observation that it binds directly to pre-mRNA. [provided by RefSeq, Jul 2008]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 14 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most 50 bp of the penultimate exon, not predicted to undergo nonsense mediated mRNA decay. There are 5 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 1-149923922-G-A is Pathogenic according to our data. Variant chr1-149923922-G-A is described in ClinVar as [Pathogenic]. Clinvar id is 65407.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

Pathogenic, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpSep 19, 2019For these reasons, this variant has been classified as Pathogenic. Loss-of-function variants in SF3B4 are known to be pathogenic (PMID: 22541558, 23568615). This variant has been observed in an individual affected with Nager syndrome (PMID: 23568615). ClinVar contains an entry for this variant (Variation ID: 65407). This variant is not present in population databases (ExAC no frequency). This sequence change creates a premature translational stop signal (p.Arg336*) in the SF3B4 gene. It is expected to result in an absent or disrupted protein product. -
