Variant summary
Our verdict is Likely pathogenic. The variant received 8 ACMG points: 8P and 0B. PM1PM2PP3_Strong
The NM_003000.3(SDHB):c.308T>C(p.Met103Thr) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.000000684 in 1,461,846 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. 13/22 in silico tools predict a damaging outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. M103V) has been classified as Uncertain significance.

In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt SDHB protein function. ClinVar contains an entry for this variant (Variation ID: 528728). This missense change has been observed in individual(s) with succinate dehydrogenase deficiency-related leukoencephalopathy (PMID: 26642834). This variant is not present in population databases (gnomAD no frequency). This sequence change replaces methionine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 103 of the SDHB protein (p.Met103Thr).
Hereditary cancer-predisposing syndrome Uncertain:1
The p.M103T variant (also known as c.308T>C), located in coding exon 4 of the SDHB gene, results from a T to C substitution at nucleotide position 308. The methionine at codon 103 is replaced by threonine, an amino acid with similar properties. This alteration was identified in an individual diagnosed with succinate dehydrogenase deficiency-related leukoencephalopathy (Helman G et al. Ann Neurol, 2016 Mar;79:379-86). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear.
