This sequence change falls in intron 2 of the SDHB gene. It does not directly change the encoded amino acid sequence of the SDHB protein. RNA analysis indicates that this variant induces altered splicing and may result in an absent or altered protein product. This variant is not present in population databases (gnomAD no frequency). This variant has been observed in individuals with SDHB-related disease (internal data). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 440259). Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Studies have shown that this variant results in activation of a cryptic splice site, and produces a non-functional protein and/or introduces a premature termination codon (internal data). For these reasons, this variant has been classified as Pathogenic.

The c.200+5G>C intronic variant results from a G to C substitution 5 nucleotides after coding exon 2 in the SDHB gene. This alteration has been reported in a study of 1/214 patients diagnosed with head and neck paragangliomas (Sen I et al. J Vasc Surg, 2020 05;71:1602-1612.e2), and has also been observed internally in at least one individual with a personal and/or family history that is consistent with SDHB-related disease (Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site; however, direct evidence is insufficient at this time (Ambry internal data). Based on the majority of available evidence to date, this variant is likely to be pathogenic.
