Variant summary
Our verdict is Likely pathogenic. Variant got 6 ACMG points: 6P and 0B. PM1PM2PP3_Moderate
The NM_206933.4(USH2A):c.1246G>A(p.Ala416Thr) variant causes a missense change. The variant allele was found at a frequency of 0.00000744 in 1,613,368 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. A416S) has been classified as Uncertain significance.

This variant was determined to be of uncertain significance according to ACMG Guidelines, 2015 [PMID:25741868]. -
not specified Uncertain:1
Variant summary: USH2A c.1246G>A (p.Ala416Thr) results in a non-conservative amino acid change located in the Laminin N-terminal domain profile (IPR008211) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 1.6e-05 in 250008 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1246G>A has been reported in the literature in the compound heterozygous state in at least one in individual affected with retinitis pigmentosa. These data do not allow any conclusion about variant significance. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication have been ascertained in the context of this evaluation (PMID: 32098976). ClinVar contains an entry for this variant (Variation ID: 1032620). Based on the evidence outlined above, the variant was classified as uncertain significance. -
not provided Uncertain:1
This sequence change replaces alanine with threonine at codon 416 of the USH2A protein (p.Ala416Thr). The alanine residue is highly conserved and there is a small physicochemical difference between alanine and threonine. This variant is present in population databases (rs187897763, ExAC 0.02%). This variant has not been reported in the literature in individuals affected with USH2A-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
