ACTN2 (HGNC:164): (actinin alpha 2) Alpha actinins belong to the spectrin gene superfamily which represents a diverse group of cytoskeletal proteins, including the alpha and beta spectrins and dystrophins. Alpha actinin is an actin-binding protein with multiple roles in different cell types. In nonmuscle cells, the cytoskeletal isoform is found along microfilament bundles and adherens-type junctions, where it is involved in binding actin to the membrane. In contrast, skeletal, cardiac, and smooth muscle isoforms are localized to the Z-disc and analogous dense bodies, where they help anchor the myofibrillar actin filaments. This gene encodes a muscle-specific, alpha actinin isoform that is expressed in both skeletal and cardiac muscles. One in silico tool (MutationTaster) predicts a polymorphism outcome for this variant. 4/5 splice prediction tools predict no significant impact on normal splicing. This variant was found in 1636/120960 control chromosomes (including 73 homozygotes), predominantly observed in the East Asian subpopulation at a frequency of 0.1080705 (932/8624). This frequency is about 4323 times the estimated maximal expected allele frequency of a pathogenic ACTN2 variant (0.000025), suggesting this is a common benign polymorphism found primarily in the populations of East Asian origin. This polymorphism has also been reported in HCM patients without strong evidence of causality. In addition, multiple clinical diagnostic laboratories have classified this variant as benign.
