Variant summary
Our verdict is Pathogenic. Variant got 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong
The NM_000642.3(AGL):βc.276delGβ(p.Gln92fs) variant causes a frameshift change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000248 in 1,613,618 control chromosomes in the GnomAD database, with no homozygous occurrence. Variant has been reported in ClinVar as Pathogenic (β β ). Synonymous variant affecting the same amino acid position (i.e. Q92Q) has been classified as Likely benign. Variant results in nonsense mediated mRNA decay.

Glycogen storage disease type III Pathogenic:6
Pathogenic, criteria provided, single submitter | clinical testing | Women's Health and Genetics/Laboratory Corporation of America, LabCorp | Apr 24, 2020 | Variant summary: AGL c.276delG (p.Gln92HisfsX16) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant allele was found at a frequency of 8e-06 in 250684 control chromosomes (gnomAD). c.276delG has been reported in the literature in individuals affected with Glycogen Storage Disease Type III (Lucchiari_2002, Crushell_2010). These data indicate that the variant may be associated with disease. At least one publication reports this variant results in <10% of normal AGL activity (Crushell_2010). One ClinVar submitter (evaluation after 2014) cites the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. - |

Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Dec 14, 2023 | This sequence change creates a premature translational stop signal (p.Gln92Hisfs*16) in the AGL gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in AGL are known to be pathogenic (PMID: 19299494). This variant is present in population databases (no rsID available, gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with glycogen storage disease type III (PMID: 20490926). ClinVar contains an entry for this variant (Variation ID: 371401). For these reasons, this variant has been classified as Pathogenic. - |
