Genes affected
ECHS1 (HGNC:3151): (enoyl-CoA hydratase, short chain 1) The protein encoded by this gene functions in the second step of the mitochondrial fatty acid beta-oxidation pathway. It catalyzes the hydration of 2-trans-enoyl-coenzyme A (CoA) intermediates to L-3-hydroxyacyl-CoAs. The gene product is a member of the hydratase/isomerase superfamily. It localizes to the mitochondrial matrix. Transcript variants utilizing alternative transcription initiation sites have been described in the literature. [provided by RefSeq, Jul 2008]

ACMG classification

Classification made for transcript

Verdict is Likely_pathogenic. Variant got 8 ACMG points.

PM1
In a hotspot region, there are 2 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 0 benign, 7 uncertain in NM_004092.4
PM2
Very rare variant in population databases, with high coverage;
PP3
MetaRNN computational evidence supports a deleterious effect, 0.916
PP5
Variant 10-133362894-T-C is Pathogenic according to our data. Variant chr10-133362894-T-C is described in ClinVar as [Likely_pathogenic]. Clinvar id is 692006.Status of the report is criteria_provided_single_submitter, 1 stars.

Likely pathogenic, criteria provided, single submitterclinical testingRady Children's Institute for Genomic Medicine, Rady Children's Hospital San DiegoJan 25, 2018This variant has not been previously reported in the literature to our knowledge, and it is absent from the ExAC or gnomAD population databases. Several disease causing missense variants located in the adjacent amino acids in exon 8 of this gene have been reported suggesting this region of the ECHS1 gene to be a hot spot for pathogenic mutations (PMID: 28202214, 27221955). In silico algorithms predict this change to be deleterious. Based on the overall evidence, we classified this variant as likely pathogenic. -
