Variant summary
Our verdict is Likely benign. Variant got -4 ACMG points: 2P and 6B. PM2BP4_ModerateBS2
The NM_014000.3(VCL):c.1195A>G(p.Asn399Asp) variant causes a missense change. The variant allele was found at a frequency of 0.0000329 in 152,034 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 12/21 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. N399H) has been classified as Likely benign.

Submissions by phenotype
Dilated cardiomyopathy 1W Uncertain:1
This sequence change replaces asparagine, which is neutral and polar, with aspartic acid, which is acidic and polar, at codon 399 of the VCL protein (p.Asn399Asp). This variant is present in population databases (rs377228795, gnomAD 0.007%). This variant has not been reported in the literature in individuals affected with VCL-related conditions. ClinVar contains an entry for this variant (Variation ID: 845353). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
Cardiovascular phenotype Uncertain:1
The p.N399D variant (also known as c.1195A>G), located in coding exon 10 of the VCL gene, results from an A to G substitution at nucleotide position 1195. The asparagine at codon 399 is replaced by aspartic acid, an amino acid with highly similar properties. This amino acid position is conserved. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
