BMPR1A (HGNC:1076): (bone morphogenetic protein receptor type 1A) The bone morphogenetic protein (BMP) receptors are a family of transmembrane serine/threonine kinases that include the type I receptors BMPR1A and BMPR1B and the type II receptor BMPR2. These receptors are also closely related to the activin receptors, ACVR1 and ACVR2. The ligands of these receptors are members of the TGF-beta superfamily. TGF-betas and activins transduce their signals through the formation of heteromeric complexes with 2 different types of serine (threonine) kinase receptors: type I receptors of about 50-55 kD and type II receptors of about 70-80 kD. Type II receptors bind ligands in the absence of type I receptors, but they require their respective type I receptors for signaling, whereas type I receptors require their respective type II receptors for ligand binding. The p.S389I variant (also known as c.1166G>T), located in coding exon 8 of the BMPR1A gene, results from a G to T substitution at nucleotide position 1166. The amino acid change results in serine to isoleucine at codon 389, an amino acid with dissimilar properties. However, this change occurs in the last base pair of coding exon 8, which makes it likely to have some effect on normal mRNA splicing. This alteration has been reported in 1/1197 individuals from Greece, Romania, and Turkey undergoing evaluation for inherited cancer predisposition (Tsaousis GN et al. BMC Cancer, 2019 Jun;19:535). This nucleotide position is highly conserved in available vertebrate species. This amino acid position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will not have any significant effect on splicing. In addition, this alteration is predicted to be deleterious by in silico protein analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -

Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). RT-PCR analysis observed no change in length of RNA produced due to splicing effects, however, Sanger sequencing revealed RNA containing the variant was absent (PMID: 31159747). Other RNA analysis did not observe an impact on splicing (ClinVar SCV000632684.7). This variant has been reported in an individual affected with breast cancer in the literature (PMID: 31159747). This variant has been identified in 1/31398 chromosomes in the general population by the Genome Aggregation Database (gnomAD). This variant is present in population databases (no rsID available, gnomAD 0.007%). This missense change has been observed in individual(s) with BMPR1A-related conditions (PMID: 31159747). ClinVar contains an entry for this variant (Variation ID: 460474). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive.
