The p.R494* pathogenic mutation (also known as c.1480C>T), located in coding exon 11 of the BMPR1A gene, results from a C to T substitution at nucleotide position 1480. This changes the amino acid from an arginine to a stop codon within coding exon 11. This mutation has been identified in individuals with a history of juvenile and hamartomatous colon polyps (Ngeow J et al. Gastroenterology 2013 Jun;144:1402-9; Ambry internal data). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation. As such, this alteration is interpreted as a disease-causing mutation. -
This variant changes 1 nucleotide in exon 13 of the BMPR1A gene, creating a premature translation stop signal in the last coding exon. This variant causes the deletion of the last 39 amino acids from the BMPR1A protein, including the C-terminal portion of the kinase domain (PMID: 8397373). To our knowledge, functional studies have not been reported for this variant. This variant has been observed in two individuals affected with juvenile polyposis (PMID: 23399955). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Loss of BMPR1A function is a known mechanism of disease (clinicalgenome.org). Based on the available evidence, this variant is classified as Pathogenic. -

RNA analysis performed to evaluate the impact of this premature translational stop signal on mRNA splicing indicates it does not significantly alter splicing (Invitae). This variant disrupts a region of the BMPR1A protein in which other variant(s) (p.Trp504*) have been determined to be pathogenic (Invitae). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic. -
