Genes affected
ALDH18A1 (HGNC:9722): (aldehyde dehydrogenase 18 family member A1) This gene is a member of the aldehyde dehydrogenase family and encodes a bifunctional ATP- and NADPH-dependent mitochondrial enzyme with both gamma-glutamyl kinase and gamma-glutamyl phosphate reductase activities. The encoded protein catalyzes the reduction of glutamate to delta1-pyrroline-5-carboxylate, a critical step in the de novo biosynthesis of proline, ornithine and arginine. Mutations in this gene lead to hyperammonemia, hypoornithinemia, hypocitrullinemia, hypoargininemia and hypoprolinemia and may be associated with neurodegeneration, cataracts and connective tissue diseases. Alternatively spliced transcript variants, encoding different isoforms, have been described for this gene. [provided by RefSeq, Jul 2008]

Likely pathogenic, no assertion criteria providedclinical testingPreventionGenetics, part of Exact SciencesApr 12, 2024The ALDH18A1 c.383G>A variant is predicted to result in the amino acid substitution p.Arg128His. This missense change was documented in the biallelic state in multiple patients with hereditary spastic paraplegia (Kalmár et al. 2021. PubMed ID: 32798076; Coutelier et al. 2015. PubMed ID: 26026163; Koh et al. 2018. PubMed ID: 29915212). This variant is reported in 0.0065% of alleles in individuals of South Asian descent in gnomAD. This variant is interpreted as likely pathogenic. -

Pathogenic, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpNov 22, 2023This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 128 of the ALDH18A1 protein (p.Arg128His). This variant is present in population databases (rs768323248, gnomAD 0.01%). This missense change has been observed in individuals with clinical features of autosomal recessive hereditary spastic paraplegia (PMID: 26026163, 29915212, 32798076). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 217121). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on ALDH18A1 protein function. For these reasons, this variant has been classified as Pathogenic. -
