The c.407G>T (p.C136F) alteration is located in exon 2 (coding exon 2) of the KCNQ1 gene. This alteration results from a G to T substitution at nucleotide position 407, causing the cysteine (C) at amino acid position 136 to be replaced by a phenylalanine (F). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear.
Cardiac arrhythmia Uncertain:1
This missense variant replaces cysteine with phenylalanine at codon 136 of the KCNQ1 protein. This variant is found within the highly conserved transmembrane domain S1 (a.a. 122-142). Rare non-truncating variants in this region have been shown to be significantly overrepresented in individuals with long QT syndrome (PMID: 32893267). Computational prediction suggests that this variant may have deleterious impact on protein structure and function. A functional study has shown that this variant causes a reduction of channel current and a faster deactivation in gating kinetics in transferred cells (PMID: 30571187). This variant has been reported in an individual suspected of having long QT syndrome (PMID: 15840476) and in another individual affected with unspecified cardiovascular disease (PMID: 31696929). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
