This sequence change replaces alanine, which is neutral and non-polar, with glutamic acid, which is acidic and polar, at codon 93 of the MYL2 protein (p.Ala93Glu). This variant is present in population databases (no rsID available, gnomAD 0.007%). This missense change has been observed in individuals with clinical features of hypertrophic cardiomyopathy (Invitae). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 858021). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. This variant disrupts the p.Ala93 amino acid residue in MYL2. Other variant(s) that disrupt this residue have been observed in individuals with MYL2-related conditions (Invitae), which suggests that this may be a clinically significant amino acid residue. For these reasons, this variant has been classified as Pathogenic.

The p.Ala93Glu variant in MYL2 has not been previously reported in individuals with cardiomyopathy, but has been identified in 0.001% (1/129178) of European chromosomes by gnomAD (http://gnomad.broadinstitute.org). Computational prediction tools and conservation analysis suggest that this variant may impact the protein, though this information is not predictive enough to determine pathogenicity. Furthermore, a different variant at the same position (p.Ala93Val) has been reported in at least one individual with hypertrophic cardiomyopathy (Berge 2014); however, there is insufficient evidence to support pathogenicity for this alternate variant.
