The POLE c.5940G>A (p.Trp1980Ter) change is a nonsense variant that is predicted to cause premature protein truncation. The disease mechanism for replication-repair-associated DNA polymerases is loss of proofreading caused by missense changes in the exonuclease domain, and protein truncating variants do not have an established correlation to disease (PMID: 23447401). This variant does not affect the exonuclease domain. This variant has a maximum subpopulation frequency of 0.0016% in gnomAD v2.1.1 (https://gnomad.broadinstitute.org/). To our knowledge, this variant has not been reported in individuals with POLE-related disease. In summary, this variant meets criteria to be classified as of uncertain significance based on the ACMG/AMP criteria: no criteria met. -

This variant has been reported in the compound heterozygous state in an individual with autosomal recessive intrauterine growth retardation, metaphyseal dysplasia, adrenal hypoplasia congenita, genital anomalies, and immunodeficiency (IMAGe-I) syndrome (Table 1, Logan CV et al. 2018. PubMed ID: 30503519). This variant is reported in 0.0016% of alleles in individuals of European (Non-Finnish) descent in gnomAD (http://gnomad.broadinstitute.org/variant/12-133210836-C-T). The p.W1980* variant (also known as c.5940G>A), located in coding exon 43 of the POLE gene, results from a G to A substitution at nucleotide position 5940. This changes the amino acid from a tryptophan to a stop codon within coding exon 43. This region is well conserved in available vertebrate species. This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. However, loss of function via haploinsufficiency in POLE has not been clearly established as a mechanism of disease. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
