Variant summary

Our verdict is Pathogenic. The variant received 18 ACMG points: 18P and 0B. PVS1_StrongPS3PM2PP5_Very_Strong

The NM_000217.3(KCNA1):c.1249C>T(p.Arg417*) variant causes a stop gained change. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). ClinVar reports functional evidence for this variant: "SCV001234273: Experimental studies have shown that this premature translational stop signal affects KCNA1 function (PMID:11026449, 19779067)." and additional evidence is available in ClinVar. Synonymous variant affecting the same amino acid position (i.e. R417R) has been classified as Likely benign.

Genes affected
KCNA1 (HGNC:6218): (potassium voltage-gated channel subfamily A member 1) This gene encodes a voltage-gated delayed potassium channel that is phylogenetically related to the Drosophila Shaker channel. The encoded protein has six putative transmembrane segments (S1-S6), and the loop between S5 and S6 forms the pore and contains the conserved selectivity filter motif (GYGD). The functional channel is a homotetramer. The N-terminus of the channel is associated with beta subunits that can modify the inactivation properties of the channel as well as affect expression levels. The C-terminus of the channel is complexed to a PDZ domain protein that is responsible for channel targeting. Mutations in this gene have been associated with myokymia with periodic ataxia (AEMK). ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 18 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most exon, not predicted to undergo nonsense mediated mRNA decay. Fraction of 0.161 CDS is truncated, and there are 1 pathogenic variants in the truncated region.
PS3
PS3 evidence extracted from ClinVar submissions: SCV001234273: Experimental studies have shown that this premature translational stop signal affects KCNA1 function (PMID: 11026449, 19779067).; SCV006301833: Published functional studies suggest a damaging effect on Kv1.1 channel function (PMID: 11026449, 19779067, 11773313, 17520476)
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 12-4912627-C-T is Pathogenic according to our data. Variant chr12-4912627-C-T is described in ClinVar as Pathogenic. ClinVar VariationId is 13491.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.
