Variant summary
Our verdict is Uncertain significance. The variant received 2 ACMG points: 2P and 0B. PM2
The NM_002234.4(KCNA5):c.898G>T(p.Gly300Cys) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000205 in 1,460,084 control chromosomes in the GnomAD database, with no homozygous occurrence. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. G300S) has been classified as Likely benign.

Inborn genetic diseases Uncertain:1
The c.898G>T (p.G300C) alteration is located in exon 1 (coding exon 1) of the KCNA5 gene. This alteration results from a G to T substitution at nucleotide position 898, causing the glycine (G) at amino acid position 300 to be replaced by a cysteine (C). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear.
Atrial fibrillation, familial, 7 Uncertain:1
In summary, this variant is a novel missense change with uncertain impact on protein function. It has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). This variant is not present in population databases (ExAC no frequency) and has not been reported in the literature in individuals with a KCNA5-related disease. This sequence change replaces glycine with cysteine at codon 300 of the KCNA5 protein (p.Gly300Cys). The glycine residue is moderately conserved and there is a large physicochemical difference between glycine and cysteine.
