Genes affected
BRCA2 (HGNC:1101): (BRCA2 DNA repair associated) Inherited mutations in BRCA1 and this gene, BRCA2, confer increased lifetime risk of developing breast or ovarian cancer. Both BRCA1 and BRCA2 are involved in maintenance of genome stability, specifically the homologous recombination pathway for double-strand DNA repair. The largest exon in both genes is exon 11, which harbors the most important and frequent mutations in breast cancer patients. The BRCA2 gene was found on chromosome 13q12.3 in human. The BRCA2 protein contains several copies of a 70 aa motif called the BRC motif, and these motifs mediate binding to the RAD51 recombinase which functions in DNA repair. BRCA2 is considered a tumor suppressor gene, as tumors with BRCA2 mutations generally exhibit loss of heterozygosity (LOH) of the wild-type allele. [provided by RefSeq, May 2020]

This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. It had not been previously curated by ICSL or reported in the Human Gene Mutation Database (HGMD: prior to June 1st, 2018), and was therefore a candidate for classification through an automated scoring system. Utilizing variant allele frequency, disease prevalence and penetrance estimates, and inheritance mode, an automated score was calculated to assess if this variant is too frequent to cause the disease. Based on the score, this variant could not be ruled out of causing disease and therefore its association with disease required further investigation. A literature search was performed for the gene, cDNA change, and amino acid change (if applicable). No publications were found based on this search. This variant was therefore classified as a variant of unknown significance for this disease. -

The p.S973L variant (also known as c.2918C>T), located in coding exon 10 of the BRCA2 gene, results from a C to T substitution at nucleotide position 2918. The serine at codon 973 is replaced by leucine, an amino acid with dissimilar properties. This alteration has been reported in individuals with breast and/or ovarian cancer (Ahmad J et al. Clin. Genet. 2012 Dec;82:594-8; Wang J et al. Cancer Med. 2019 May;8(5):2074-2084; Guo X et al. Int J Cancer. 2020 Apr;146(8):2175-2181; Guindalini RSC et al. Sci Rep. 2022 Mar;12(1):4190; Yao L et al. J Hum Genet. 2022 Nov;67(11):639-642). This variant was also reported in 4/60,466 breast cancer cases and in 2/53,461 controls (Dorling et al. N Engl J Med. 2021 02;384:428-439). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Variant summary: BRCA2 c.2918C>T (p.Ser973Leu) results in a non-conservative amino acid change in the encoded protein sequence. Three of four in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4.1e-06 in 243890 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.2918C>T has been reported in the literature in an individual affected with Hereditary Breast and Ovarian Cancer (Ahmad_2012). This report does not provide an unequivocal conclusion about association of the variant with Hereditary Breast and Ovarian Cancer. A clinical lab via ClinVar has reported one patient who also carries an unspecified pathogenic BRCA1 mutation, suggesting the benign role of the variant of interest. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Five ClinVar submissions from other clinical diagnostic laboratories (evaluation after 2014) cites the variant as "uncertain significance." Based on the evidence outlined above, the variant was classified as uncertain significance. -

The BRCA2 c.2918C>T (p.Ser973Leu) variant has been reported in the published literature in individuals with breast cancer (PMIDs: 35864222 (2022), 35264596 (2022), 33471991 (2021) see also LOVD (https://databases.lovd.nl/shared/), 31837001 (2020), 30982232 (2019), 22486713 (2012)) and reportedly healthy individuals (PMID: 33471991 (2021) see also LOVD (https://databases.lovd.nl/shared/)). Additionally, this variant has been reported to be located in a region of the BRCA2 gene that is tolerant to missense sequence changes (PMID: 31911673 (2020)). The frequency of this variant in the general population, 0.000004 (1/248678 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. -
