This sequence change replaces phenylalanine, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 1898 of the MYH6 protein (p.Phe1898Leu). This variant is present in population databases (rs201199853, gnomAD 0.005%). This missense change has been observed in individual(s) with dilated cardiomyopathy (PMID: 30847666). ClinVar contains an entry for this variant (Variation ID: 537945). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt MYH6 protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
Cardiovascular phenotype Uncertain:1
The p.F1898L variant (also known as c.5694C>G), located in coding exon 36 of the MYH6 gene, results from a C to G substitution at nucleotide position 5694. The phenylalanine at codon 1898 is replaced by leucine, an amino acid with highly similar properties. This alteration has been reported in a cardiomyopathy genetic testing cohort; however, clinical details were limited, and additional cardiac variants were detected in some cases (van Lint FHM et al. Neth Heart J, 2019 Jun;27:304-309). In addition, a different alteration located at the same position, resulting in the same protein change, c.5694C>A (p.F1898L), has been reported in a rhabdomyolysis cohort (Kruijt N et al. Eur J Neurol, 2021 Feb;28:647-659). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
