The p.Glu1403Lys in MYH6 has not been previously reported in individuals with ca rdiomyopathy, but has been identified in 1/8758 East Asian chromosomes by the Ex ome Aggregation Consortium (ExAC, http://exac.broadinstitute.org). Computational prediction tools and conservation analysis suggest that this variant may impact the protein, though this information is not predictive enough to determine path ogenicity. In summary, the clinical significance of the p.Glu1403Lys variant is uncertain.

Variant summary: MYH6 c.4207G>A (p.Glu1403Lys) results in a conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change. The variant allele was found at a frequency of 3.2e-05 in 251394 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.4207G>A has been observed in individual(s) affected with hypertrophic cardiomyopath and sudden unexplained nocturnal death syndrome, without strong evidence for causality (Chung_2021, Zhang_2016). These report(s) do not provide unequivocal conclusions about association of the variant with Cardiomyopathy. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 33658040, 27707468). ClinVar contains an entry for this variant (Variation ID: 228894). This sequence change replaces glutamic acid, which is acidic and polar, with lysine, which is basic and polar, at codon 1403 of the MYH6 protein (p.Glu1403Lys). This variant is present in population databases (rs769398160, gnomAD 0.006%). This variant has not been reported in the literature in individuals affected with MYH6-related conditions. ClinVar contains an entry for this variant (Variation ID: 228894). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt MYH6 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

The MYH6 c.4207G>A variant is predicted to result in the amino acid substitution p.Glu1403Lys. This variant was reported as a variant of uncertain significance in an individual with hypertrophic cardiomyopathy (Table S1, Chung et al. 2021. PubMed ID: 33658040) and an individual with sudden unexplained nocturnal death syndrome (Table S2, Zhang et al. 2016. PubMed ID: 27707468). This variant is reported in 0.0054% of alleles in individuals of East Asian descent in gnomAD (http://gnomad.broadinstitute.org/variant/14-23857516-C-T). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence.

The p.E1403K variant (also known as c.4207G>A), located in coding exon 28 of the MYH6 gene, results from a G to A substitution at nucleotide position 4207. The glutamic acid at codon 1403 is replaced by lysine, an amino acid with similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
