This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 1384 of the MYH6 protein (p.Arg1384Trp). This variant is present in population databases (rs763926580, gnomAD 0.003%). This variant has not been reported in the literature in individuals affected with MYH6-related conditions. ClinVar contains an entry for this variant (Variation ID: 407149). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt MYH6 protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Our patient has dilated left ventricle and prominent trabeculations. The lab classifies this variant as a variant of unknown significance. Given a lack of case data we consider this variant a variant of unknown significance and we do not feel it is suitable for assessing risk in healthy relatives ("predictive genetic testing"). This variant has not been reported with disease before. In silico analysis with PolyPhen-2 predicts the variant to be probably damaging (HumVar: 1.00). The argenine at codon 1384 is conserved across species, as are neighboring amino acids. Other variants have not been reported in association with disease at this codon (1384) although missense variants of unknown significance have been identified nearby codons (Thr1379Met and Gly1389Lys). There is one individual with p.Arg1384Trp listed in the Exome Aggregation Consortium Dataset (ExAC), which currently includes variant calls on ~60,000 unrelated individuals of African, Asian, European, and Latino descent. -

The p.R1384W variant (also known as c.4150C>T), located in coding exon 27 of the MYH6 gene, results from a C to T substitution at nucleotide position 4150. The arginine at codon 1384 is replaced by tryptophan, an amino acid with dissimilar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
