MYH7 (HGNC:7577): (myosin heavy chain 7) Muscle myosin is a hexameric protein containing 2 heavy chain subunits, 2 alkali light chain subunits, and 2 regulatory light chain subunits. This gene encodes the beta (or slow) heavy chain subunit of cardiac myosin. It is expressed predominantly in normal human ventricle. It is also expressed in skeletal muscle tissues rich in slow-twitch type I muscle fibers. Changes in the relative abundance of this protein and the alpha (or fast) heavy subunit of cardiac myosin correlate with the contractile velocity of cardiac muscle. Its expression is also altered during thyroid hormone depletion and hemodynamic overloading. Mutations in this gene are associated with familial hypertrophic cardiomyopathy, myosin storage myopathy, dilated cardiomyopathy, and Laing distal myopathy. [provided by RefSeq, May 2022]

The variant of interest has been found in a large, broad control population, ExAC in 2/121412 control chromosomes at a frequency of 0.0000165, which does not exceed the estimated maximal expected allele frequency of a pathogenic MYH7 variant (0.0010005). In addition, one clinical diagnostic laboratories/reputable databases classified this variant as uncertain significance. Taken together, this variant is classified as a VUS, until more clinical and functional information becomes available. -

This variant is present in population databases (rs187073962, gnomAD 0.006%). This missense change has been observed in individual(s) with dilated cardiomyopathy and/or MYH7-related conditions (PMID: 32880476, 37652022). ClinVar contains an entry for this variant (Variation ID: 181289). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt MYH7 protein function with a positive predictive value of 95%. The p.E1902K variant (also known as c.5704G>A), located in coding exon 37 of the MYH7 gene, results from a G to A substitution at nucleotide position 5704. The glutamic acid at codon 1902 is replaced by lysine, an amino acid with similar properties. This alteration has been reported in a hypertrophic cardiomyopathy (HCM) cohort and a dilated cardiomyopathy (DCM) cohort; however, clinical details were limited (Mazzarotto F et al. Genet Med, 2019 02;21:284-292; Verdonschot JAJ et al. Circ Genom Precis Med, 2020 10;13:476-487). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

The MYH7 c.5704G>A variant is predicted to result in the amino acid substitution p.Glu1902Lys. This variant was reported in a few individuals with dilated or hypertrophic cardiomyopathy (Table S2, Zimmerman et al 2010. PubMed ID: 20474083; Table S4, Verdonschot et al 2020. PubMed ID: 32880476; Table S3, Mazzarotto et al. 2019. PubMed ID: 29875424). This variant is reported in 0.0054% of alleles in individuals of East Asian descent in gnomAD (http://gnomad.broadinstitute.org/variant/14-23883054-C-T). A different variant affecting the same amino acid (Glu1902Gln) was reported to be associated with hypertrophic cardiomyopathy/Brugada syndrome (Human Gene Mutation Database; Chen et al. 2022. PubMed ID: 36303204). In ClinVar, this variant is interpreted as uncertain (https://preview.ncbi.nlm.nih.gov/clinvar/variation/181289/). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -
