In summary, this is a novel missense change with uncertain impact on splicing. It has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may alter RNA splicing, but this prediction has not been confirmed by published transcriptional studies. A computational algorithm designed to assess the pathogenicity of variants in MYH7 with regard to hypertrophic cardiomyopathy predicted this sequence change to be tolerated. The algorithm has a sensitivity of 94% and a specificity of 89% (PMID: 21310275). This variant is not present in population databases (ExAC no frequency) and has not been reported in the literature in individuals with a MYH7-related disease. This sequence change replaces aspartic acid with asparagine at codon 299 of the MYH7 protein (p.Asp299Asn). The aspartic acid residue is highly conserved and there is a small physicochemical difference between aspartic acid and asparagine. It also falls at the last nucleotide of exon 10 of the MYH7 coding sequence. -
