Genes affected
FOXG1 (HGNC:3811): (forkhead box G1) This locus encodes a member of the fork-head transcription factor family. The encoded protein, which functions as a transcriptional repressor, is highly expressed in neural tissues during brain development. Mutations at this locus have been associated with Rett syndrome and a diverse spectrum of neurodevelopmental disorders defined as part of the FOXG1 syndrome. This gene is disregulated in many types of cancer and is the target of multiple microRNAs that regulate the proliferation of tumor cells. [provided by RefSeq, Jul 2020]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 18 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most exon, not predicted to undergo nonsense mediated mRNA decay. There are 59 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 14-28767535-C-T is Pathogenic according to our data. Variant chr14-28767535-C-T is described in ClinVar as [Likely_pathogenic]. Clinvar id is 189611.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars. Variant chr14-28767535-C-T is described in Lovd as [Pathogenic]. Variant chr14-28767535-C-T is described in Lovd as [Pathogenic].

Pathogenic, criteria provided, single submitterclinical testingGeneDxFeb 05, 2015The Q86X nonsense mutation in the FOXG1 gene has been identified previously in a patient with clinical features consistent with congenital Rett syndrome (Kortum et al., 2011). This mutation is predicted to cause loss of normal protein function through protein truncation. Although this mutation has not been reported previously to our knowledge, its presence is consistent with a diagnosis of a FOXG1-related disorder. The variant is found in FOXG1 panel(s). -

Based on the classification scheme defined by the ClinGen Rett/Angelman-like Expert Panel for Rett/AS-like Disorders Specifications to the ACMG/AMP Variant Interpretation Guidelines VCEP 3.0, this variant is classified as pathogenic. At least the following criteria are met: Predicted to result in loss of function, and LOF is a known mechanism of disease (PVS1). This variant is absent from gnomAD v4 (PM2_Supporting). Has been observed in at least 2 individuals with phenotypes consistent with FOXG1 disorder (PS4_Supporting). PMID:28661489 PMID:28661489 , Variation ID: 189611 -
