Genes affected
FBN1 (HGNC:3603): (fibrillin 1) This gene encodes a member of the fibrillin family of proteins. The encoded preproprotein is proteolytically processed to generate two proteins including the extracellular matrix component fibrillin-1 and the protein hormone asprosin. Fibrillin-1 is an extracellular matrix glycoprotein that serves as a structural component of calcium-binding microfibrils. These microfibrils provide force-bearing structural support in elastic and nonelastic connective tissue throughout the body. Asprosin, secreted by white adipose tissue, has been shown to regulate glucose homeostasis. Mutations in this gene are associated with Marfan syndrome and the related MASS phenotype, as well as ectopia lentis syndrome, Weill-Marchesani syndrome, Shprintzen-Goldberg syndrome and neonatal progeroid syndrome. Pathogenic, criteria provided, single submitter | clinical testing | Laboratory for Molecular Medicine, Mass General Brigham Personalized Medicine | Apr 16, 2019 | The p.Arg2057X variant in FBN1 has been reported in 1 individual with Marfan syndrome and 1 individual with primary spontaneous pneumothorax (Liu 1997, Schrijver 2002, Zhang 2016). It was absent from large population studies. This variant has also been reported in ClinVar (Variation ID 200076). This nonsense variant leads to a premature termination codon at position 2057, which is predicted to lead to a truncated or absent protein. Loss of function of the FBN1 gene is an established disease mechanism in autosomal dominant Marfan syndrome. In summary, this variant meets criteria to be classified as pathogenic for autosomal dominant Marfan syndrome. ACMG/AMP Criteria applied: PVS1, PS4_Supporting, PM2. - |

Pathogenic, criteria provided, single submitter | clinical testing | Women's Health and Genetics/Laboratory Corporation of America, LabCorp | Dec 12, 2022 | Variant summary: FBN1 c.6169C>T (p.Arg2057X) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant was absent in 251028 control chromosomes (gnomAD). c.6169C>T has been reported in the literature in individuals affected with Marfan Syndrome (example: Mannucci_2020 and Schrijver_2002). These data indicate that the variant is likely to be associated with disease. Four clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 and all classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. - |

Pathogenic, criteria provided, single submitter | clinical testing | GeneDx | Oct 31, 2018 | The Arg2057Stop variant in the FBN1 gene has been reported in at least one individual with Marfan syndrome (Liu W et al., 1997; Schrijver I et al., 2002). Arg2057Stop is predicted to cause loss of normal protein function either by protein truncation or nonsense-mediated mRNA decay. Other nonsense variant in the FBN1 gene have been reported in association with Marfan syndrome. The Arg2057Stop variant also has been observed in other unrelated individuals tested for Marfan syndrome at GeneDx.In summary, Arg2057Stop in the FBN1 gene is interpreted as a disease-causing variant. - |

Marfan syndrome;C4707243:Familial thoracic aortic aneurysm and aortic dissection Pathogenic:1
Pathogenic, criteria provided, single submitter | clinical testing | Invitae | Nov 04, 2022 | For these reasons, this variant has been classified as Pathogenic. ClinVar contains an entry for this variant (Variation ID: 200076). This premature translational stop signal has been observed in individual(s) with Marfan syndrome or a related connective tissue disorder and in an individual affected with primary spontaneous pneumothorax (PMID: 10464652, 27229674). This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Arg2057*) in the FBN1 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in FBN1 are known to be pathogenic (PMID: 17657824, 19293843). - |
