FBN1 (HGNC:3603): (fibrillin 1) This gene encodes a member of the fibrillin family of proteins. The encoded preproprotein is proteolytically processed to generate two proteins including the extracellular matrix component fibrillin-1 and the protein hormone asprosin. Fibrillin-1 is an extracellular matrix glycoprotein that serves as a structural component of calcium-binding microfibrils. These microfibrils provide force-bearing structural support in elastic and nonelastic connective tissue throughout the body. Asprosin, secreted by white adipose tissue, has been shown to regulate glucose homeostasis. Mutations in this gene are associated with Marfan syndrome and the related MASS phenotype, as well as ectopia lentis syndrome, Weill-Marchesani syndrome, Shprintzen-Goldberg syndrome and neonatal progeroid syndrome. A novel C982S variant that is likely pathogenic was identified in the FBN1 gene. It has not been published as apathogenic variant, nor has it been reported as a benign variant to our knowledge. The C982S variant was notobserved in approximately 6,500 individuals of European and African American ancestry in the NHLBI ExomeSequencing Project, indicating it is not a common benign variant in these populations. The C982S variant is anon-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differin polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved acrossspecies, and in silico analysis predicts this variant is probably damaging to the protein structure/function.Furthermore, a missense variant at the same residue (C982W) and in nearby residues (C980S, C980Y, C981S,S983F, V984I) have been reported in the Human Gene Mutation Database in association with Marfan syndrome(Stenson et al., 2014), supporting the functional importance of this region of the protein. Nevertheless, this variantlacks observation in a significant number of affected individuals, segregation data, and functional evidence, whichwould further clarify its pathogenicity.Therefore, this variant is likely pathogenic. In order to definitively determine its clinical significance, additional datais required. -

This sequence change replaces cysteine with serine at codon 982 of the FBN1 protein (p.Cys982Ser). The cysteine residue is highly conserved and there is a moderate physicochemical difference between cysteine and serine. This variant is not present in population databases (ExAC no frequency). This variant has been observed in an individual affected with FBN1-related disease (Invitae). ClinVar contains an entry for this variant (Variation ID: 392364). Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). This variant affects a cysteine residue located within a TGFBP domain of the FBN1 protein. Cysteine residues in these domains are believed to be involved in intramolecular disulfide bridges and to be important for FBN1 structure. Although the exact function of the FBN1 TGFBP domains has not being elucidated (PMID: 10930463, 27437668), missense substitutions within the TGFBP domains affecting cysteine residues are significantly overrepresented among patients with Marfan syndrome (PMID: 16571647, 17701892). The p.Cys982 amino acid residue in FBN1 has been determined to be clinically significant (PMID: 26787436, 28550590). This suggests that variants that disrupt this residue are likely to be causative of disease. For these reasons, this variant has been classified as Pathogenic. -
