The p.M1? pathogenic mutation (also known as c.1A>T) is located in coding exon 1 of the FBN1 gene and results from a A to T substitution at nucleotide position 1. This alters the methionine residue at the initiation codon (ATG). This variant was reported in individual(s) with features consistent with Marfan syndrome (Rybczynski M, et al. Am J Med Genet A. 2008 Dec 15;146A(24):3157-66; Söylen B, et al. Clin Genet. 2009 Mar;75(3):265-70. Epub 2009 Jan 20).This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This amino acid position is highly conserved in available vertebrate species. In addition to the clinical data presented in the literature, sequence variations that modify the initiation codon are expected to result in either loss of translation initiation, N-terminal truncation, or cause a shift in the mRNA reading frame. Based on the supporting evidence, this variant is interpreted as a disease-causing mutation.

For these reasons, this variant has been classified as Pathogenic. This variant disrupts the p.Cys89Tyr amino acid residue in FBN1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 19293843, 19720936; Invitae). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. ClinVar contains an entry for this variant (Variation ID: 577426). Disruption of the initiator codon has been observed in individuals with Marfan syndrome (PMID: 19012347; 19159394. 27906200). This variant is not present in population databases (gnomAD no frequency). This sequence change affects the initiator methionine of the FBN1 mRNA. The next in-frame methionine is located at codon 99.
