This variant is classified as a variant of uncertain significance as there is insufficient evidence to determine its impact on protein function and/or cancer risk. -

Hereditary cancer-predisposing syndrome Uncertain:2Benign:1
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -
This missense variant replaces threonine with methionine at codon 340 of the CDH1 protein. To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with hereditary cancer in the literature. This variant has been identified in 3/251490 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

not specified Uncertain:1
Variant summary: CDH1 c.1019C>T (p.Thr340Met) results in a non-conservative amino acid change located in the Cadherin-like domain of the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1.2e-05 in 251490 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1019C>T has been reported in the literature in individuals affected with cancer as a somatic variant (e.g. Endo_2001, Giannakis_2014). These reports do not provide unequivocal conclusions about association of the variant with Hereditary Diffuse Gastric Cancer. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Five clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation (Likely benign n=1, VUS n=4). Based on the evidence outlined above, the variant was classified as uncertain significance. -
not provided Uncertain:1
Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant does not alter protein structure/function; Observed in a patient with hereditary diffuse gastric cancer (Pan et al., 2021); This variant is associated with the following publications: (PMID: 11241409, 25344691, 35582588, 33471991, 32175104, 15235021, 22850631, 34537906) -
Malignant tumor of breast Uncertain:1
The CDH1 p.Thr340Met variant was not identified in the literature nor was it identified in the MutDB, database. The variant was identified in dbSNP (ID: rs61747631) as "With other allele", ClinVar (classified as likely benign by Ambry Genetics; as uncertain significance by Invitae, GeneDx, Color Genomics, Counsyl), Cosmic (3x in biliary tract or large intestine), and in Zhejiang University databases. The variant was identified in control databases in 3 of 246268 chromosomes at a frequency of 0.00001 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the African in 2 of 15304 chromosomes (freq: 0.0001), European in 1 of 22300 chromosomes (freq: 0.00005), while the variant was not observed in the Other, Latino, European, Ashkenazi Jewish, East Asian, and South Asian populations. The p.Thr340 residue is not conserved in mammals and four out of five computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) do not suggest a high likelihood of impact to the protein; however, this information is not predictive enough to rule out pathogenicity. The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, HumanSpliceFinder) do not predict a difference in splicing. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of uncertain significance. -
CDH1-related diffuse gastric and lobular breast cancer syndrome Benign:1
The c.1019C>T (p.Thr340Met) missense variant has a frequency of 0.0123% in Africans (2 of 16256 alleles) in the gnomAD v2.1.1 cohort.
