Variant summary
Our verdict is Pathogenic. Variant got 11 ACMG points: 11P and 0B. PM1PM2PM5PP2PP3_ModeratePP5_Moderate
The ENST00000358273.9(NF1):c.4318A>G(p.Lys1440Glu) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. K1440Q) has been classified as Likely pathogenic.

Neurofibromatosis, type 1 Pathogenic:1
Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Feb 17, 2016 | This sequence change replaces lysine with glutamic acid at codon 1419 of the NF1 protein (p.Lys1419Glu). The lysine residue is moderately conserved and there is a small physicochemical difference between lysine and glutamic acid. This variant is not present in population databases (ExAC no frequency). This variant has been reported in the literature in an individual suspected with neurofibromatosis. However, this individual did not meet clinical criteria for the disease (PMID: 15060124). Family studies indicate this missense variant likely was not inherited from either parent and occurred de novo in proband with multiple cafe au lait spots, neurofibromas, and lisch nodules. In summary, this variant has not been observed in the population databases, but observed in individuals with suspected neurofibromatosis. It was confirmed to have arisen de novo through family testing in an affected individual. For these reasons, this variant has been classified as Pathogenic. - |
