BRCA1 (HGNC:1100): (BRCA1 DNA repair associated) This gene encodes a 190 kD nuclear phosphoprotein that plays a role in maintaining genomic stability, and it also acts as a tumor suppressor. The BRCA1 gene contains 22 exons spanning about 110 kb of DNA. The encoded protein combines with other tumor suppressors, DNA damage sensors, and signal transducers to form a large multi-subunit protein complex known as the BRCA1-associated genome surveillance complex (BASC). This gene product associates with RNA polymerase II, and through the C-terminal domain, also interacts with histone deacetylase complexes. This protein thus plays a role in transcription, DNA repair of double-stranded breaks, and recombination. Mutations in this gene are responsible for approximately 40% of inherited breast cancers and more than 80% of inherited breast and ovarian cancers. Alternative splicing plays a role in modulating the subcellular localization and physiological function of this gene. Many alternatively spliced transcript variants, some of which are disease-associated mutations, have been described for this gene, but the full-length natures of only some of these variants has been described. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). Splice site prediction tools suggest that this variant may not impact RNA splicing. To our knowledge, functional studies have not been performed for this variant. This variant has been detected in a breast cancer case-control meta-analysis in 2/60466 cases and 0/53461 unaffected individuals (PMID: 33471991; Leiden Open Variation Database DB-ID BRCA1_002535) and in a suspected hereditary breast and ovarian cancer family (PMID: 9333265). This variant also has been reported in a mother-daughter pair both affected with high-risk breast cancer and a BRCA2 truncation variant (PMID: 30214756). This variant also has been reported with co-occurrence and family history likelihood ratios for pathogenicity of 1.1026 and 0.0608, respectively (PMID: 31131967). The p.A1142P variant (also known as c.3424G>C), located in coding exon 9 of the BRCA1 gene, results from a G to C substitution at nucleotide position 3424. The alanine at codon 1142 is replaced by proline, an amino acid with highly similar properties. This alteration has been reported in a Turkish woman with a personal history of bilateral triple negative breast cancer diagnosed at age 33 and a family history of early-onset breast cancer in her mother. This alteration was seen in conjunction with a BRCA2 truncating alteration, classified as pathogenic, and an ATM missense alteration, classified as uncertain significance (Celik E et al. Clin Case Rep, 2018 Sep;6:1751-1755). This alteration has also been reported in 1/798 individuals from a multi-center study of persons thought to be at elevated a priori risk for a BRCA1 mutation based on personal and/or family history (Shattuck-Eidens D et al. JAMA, 1997 Oct;278:1242-50). This variant was reported in 2/60,466 breast cancer cases and in 0/53,461 controls (Dorling et al. N Engl J Med 2021 02;384:428-439). This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

The BRCA1 c.3424G>C (p.Ala1142Pro) variant has been reported in the published literature in individuals with breast and/or ovarian cancer (PMIDs: 9333265 (1997), 33471911 (2021) see also LOVD (https://databases.lovd.nl/shared/variants/BRCA1)). However, this variant was also seen in an individual with early-onset triple negative breast cancer who carried an additional pathogenic variant in BRCA2, suggesting the current variant may not be responsible for disease (PMID: 30214756 (2018)). This variant has not been reported in large, multi-ethnic general populations (Genome Aggregation Database, http://gnomad.broadinstitute.org). Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is deleterious or benign. Based on the available information, we are unable to determine the clinical significance of this variant. -
