The valine at codon 241 is replaced by methionine, an amino acid with highly similar properties. This alteration has been detected in a patient diagnosed with acute myeloid leukemia and in a patient diagnosed with sarcoma (Lu C et al. Nat Commun. 2015 Dec;6:10086; Ballinger ML et al. Lancet Oncol. 2016 Sep;17:1261-71). This variant was also reported in 1/60,466 breast cancer cases and in 3/53,461 controls (Dorling et al. N Engl J Med. 2021 02;384:428-439). This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.721G>A has been reported in the literature in an individual affected with AML (Lu_2015), but has not to our knowledge been reported in patients with Hereditary Breast And Ovarian Cancer Syndrome. This report does not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Seven clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation.
